Fall 2011 Class Schedule
Fall program begins Aug. 22nd!
Registration Fee:
A registration fee of $40 New Students & $30 Returning Students is required at the time of registration.
**Registration fee is valid for 1 school year. Additional registration fee is required for summer semester. **
Class Sizes: (Preschoolers up to 6yrs.) 8-10 students / (7yrs. & up) 12-15 students Recital Fees: $180.00 or $135.00
Class Fees: (Fees are per student) 1 class: $55.00 monthly (30 min.) **Pilates -$75.00 monthly (2 days a wk)
1 Class: $65.00 monthly (45 min.) 1 Class: $70.00 monthly (60 min.) 2 Classes: $120.00 monthly
3 Classes: $150.00 monthly 4 Classes: $200.00 monthly 5 Classes: $235.00 monthly
6 Classes: $245.00 monthly 7 Classes: 265.00 monthly Unlimited: $275.00 monthly
**Sibling Discount: For a second child that is enrolled in the same class, the discounted tuition rate is $40.00 per month. **
Trial Class: $20.00 (Register with in 2 days and payment is credited back to account.) Trial Month: $50.00 (1 month only)
Private Fees: 30 min. /$50.00, 45 min. /$75.00, 60 min. /$100.00 (must schedule in advance)
Open Studio Time: 30 min. /$15.00, 45 min. /$20.00, 60 min. /$30.00 (must schedule in advance)
Tuition Payments: All tuition payments MUST be paid by ONE OF THREE OPTIONS:
1.) Pay by semester NON-REFUNDABLE: Payment can be done by check, cash, or credit card. Fall Semester: August 30th -December (4 months) Spring Semester: January-June (5 1/2 months) 2.) Automatic monthly withdrawals from credit card or debit card: At the first of each month, tuition will be debited from your account. A convenience fee of $1.50 will be charged to your card each month. Credit card or bank debit payments are not accepted in-person on a monthly basis. 3.) Postdated Checks: Simply write a check for each month and date checks for the 1st of each month. All checks must be received upon registration to confirm enrollment. No class spot will be confirmed without receipt of all postdated checks. Failure to provide postdated checks will result in an extra fee of $15.00 per month.
** Payment Authorization Sheet: must be signed in order to release account information to PDF. Form needs to be signed & returned at the time of registration. **
General Payment Policies:
1. All tuition fees will be processed at the 1st of each month; any payment that is not honored by financial institution will incur a late fee $15 if not reconciled by the 10th of the current month.
2. All returned checks will have a $30.00 service charge.
3. No discounts or prorated options will be given for scheduled days off, holidays, illness, family events, or school functions.
4. For insufficient or neglected payments, student participation will be suspended until account is settled.
5. All registration fees, tuition, recital, and other payments are non-transferrable and non-refundable.
6. No monthly statements will be sent out. Only statements for late or delinquent payments will be given.
7. A receipt will be given to all payments made in cash.
***Registration Fee & 1ST month’s tuition is due at the time of registration - NO EXCEPTIONS***
Withdrawal/Refunds:
There is a two month minimum for all lessons. One month notice from the first month is required to discontinue any classes. Withdrawal must be done in person and will not be accepted over the phone. No withdrawals will be accepted after April 1st. To withdraw, a parent or adult must complete the following steps:
1. Inform the studio in person 2. Complete and sign a withdrawal form provided by PDF
*All unprocessed checks, dated after the one month period, will be returned. All automatic bank debiting or credit card charges will stop after the one month period. Prosper Dance Force reserves the right to refuse or terminate lessons to any student without notice. In such a case, a refund for unused lessons will be given.
* If a refund request is made for any reason, a $50.00 processing fee will be required.
Attendance/Tardiness:
PDF reserves the right to have students who come late to class, sit out the class. Repeated tardiness may result in the termination of lessons. A minimum attendance level is required. If a student misses more than 4 classes without written notice, the studio reserves the right to terminate lessons. Students missing more than 4 classes after the month of January will not be allowed to perform in the end of the year recital.
Make-ups:
Students are welcome to do make-ups in any class that is age appropriate. Call the front desk in order to schedule.
Parent’s Responsibility:
It is the responsibility of the parent or adult student to be aware of all studio activities, such as observation days, recitals, extra classes, and dates the studio is open/closed. PDF will post notices inside the facility as well as sending out notes home with the student. It is the parent’s responsibility to regularly check with the studio or the PDF website to ensure they are informed. It is also the responsibility of the parent or adult student to inform PDF of any address or telephone number change.
Care of Students:
PDF is not responsible for care after class for students. Parents with students under the age of 5 are to remain in the studio during classes. Students are not to be left at the studio for an excessive amount of time before or after classes.
Injuries:
Parents, legal guardians, and adult students waive the right to any legal action for any injury sustained on studio property resulting from normal activity conducted by the students before, during and after class time.
Photo Release:
PDF is hereby granted permission to take pictures of the students and use for advertisement or display purposes only.
